Multiple Provider Networks

We provide Multiple Provider Networks which allow members to use the network that best meets their specific needs. They can choose from Nationwide Vision, the SightCare Provider Network or utilize the Out-of-Network benefit options.

Frame Benefit Frequencies

• For frames, lenses, or contact lenses plan sponsors can select 12 or 24 months.

Unique Plan Enhancements Offered Through Nationwide Vision

• Zero or reduced plan CoPayments
• Frame Assortment – 400+ frames available under covered benefit when $120 plan design and above is selected. Most Private Doctors’ offices display 500 total frames of which usually only 75 are covered at 100% to the member.
• Standard Progressive lenses are covered 100% after a $30 CoPay.
• Contact Fitting Fee is covered 100% when members elect to use the contact lens benefit.
• Contact Lens Benefit goes specifically toward purchase of contacts, if contacts are selected on the initial visit. The fitting fee does not reduce the contact lens benefit. This is a significant enhancement over other plans. The average fitting fee for private doctors is $65, dramatically reducing the benefit allowance that can be used toward the purchase of lenses.
• LASIK Benefit is $200 ($100 per eye) or $150 ($75 per eye) depending on the plan design in lieu of the exam, eyeglasses or contact lenses.
• Second Pair Frame purchase is at a 50% discount
• Replacement contact lenses are covered at a 10% discount for disposable and 20% discount for conventional
